Make A Document A Pdf Fill In Form

You can convert a document into a fill in form.


A fill in form is a document that has interactive fields that can be completed by users on a computer. You can make a fill in form and have the form completed by your clients, patients or other people from whom you need to gather information. Adobe Acrobat allows you to take a form that you created in Adobe, Microsoft Word or Microsoft Excel and convert it to a PDF fill in form. You can also scan a form into Adobe and convert it to a PDF fill in form.


Instructions


Create a Form from an Electronic Document


1. Open Adobe Acrobat 9.


2. Click "Forms" > "Start Form Wizard."


3. Select "An existing electronic document" and click "Next."


4. Select the "Use the current document" option if you want to convert an Adobe file that is open in the window. If you want to find the file, select "Import a file from file system" and click "Browse" to find the file that you want to make a form and then click "Open."


5. Click "Next" to convert the file to a fill in form.


Create a Form from a Paper Document


6. Open Adobe Acrobat 9.


7. Click "Forms" > "Start Form Wizard."


8. Select "A paper form" and click "Next."


9. Specify the scan options. When you are ready to scan the document, click "Scan."


10. Select "Scan More Pages" if you have more than one page to scan.


11. Select "Scan Is Complete" when you finish scanning your document.


12. Click "Next" to convert the scanned file to a fill in form.
